Policies Agreement

In consideration of my child/children or myself, being allowed to participate in any way in the courses offered by The Dance Studio in Oliver, related events and activities of The Dance Studio in Oliver, and for other good and valuable considerations, the receipt of which is hereby acknowledged, the undersigned acknowledges, appreciates, and agrees that:

PAYMENT AND TUITION

Tuition for participation in The Dance Studio in Oliver activities is due by the 30th of each month (28th. For Feb). Unpaid accounts shall have an
additional $25.00 late fee applied monthly to the balance of the amount owing. All tuition and fees are non-refundable under any circumstances - "Acts
of God" or circumstances outside of our control are agreed to when submitting this registration.

MEDIA RELEASE

The undersigned, on behalf of themselves, heirs, assigns, personal representatives and next of kin, hereby grant and convey unto The Dance Studio in
Oliver, all of their right, title and interest in any and all photographic images, video recordings and audio recordings were taken or made by The Dance Studio
during the student’s activities with The Dance Studio in Oliver, including but not limited to any royalties, proceeds or other benefits derived from such
images or recordings.

MEDICAL AUTHORIZATION

The undersigned hereby grants The Dance Studio in Oliver, its owners, operators and its agents or employees the right to seek medical treatment for
the student if a parent or legal guardian, or designated emergency contact cannot be reached in a reasonable period of time.

UNDERSIGNED WARRANTY

The undersigned warrants that the student has no physical or mental restrictions to full participation in the program, except where full and
proper disclosure has already been made to The Dance Studio in Oliver in writing. Should the student's physical or mental condition change at any time
during the course of the activities with The Dance Studio in Oliver, the undersigned shall notify The Dance Studio in Oliver immediately and provide written confirmation of the change in circumstances.

WAIVER AND RELEASE OF LIABILITY

The undersigned fully understands and anticipates that The Dance Studio in Oliver activities may be hazardous to the student, including strenuous physical activity, and they hereby expressly assume the risk of injury or harm for the student's participation in The Dance Studio in Oliver activities. The undersigned knowingly and freely assumes such risks, both known and unknown, of the student’s participation in The Dance Studio in Oliver activities, even if arising from
the negligence of The Dance Studio in Oliver or others and assumes full responsibility for the student’s participation in those activities. The undersigned acknowledges the contagious nature of COVID-19 and other contagious diseases and viruses and voluntarily assume the risk that they or their children may
be exposed to or infected by COVID-19 by attending and participating and that such exposure or infection may result in personal injury, illness, permanent disability, and death. The undersigned understands that the risk of becoming exposed or infected by COVID-19 and other contagious diseases and viruses
may result from the actions, omissions, or negligence of the undersigned and others, including but not limited to employees, volunteers, and program
participants and their families. The undersigned, on behalf of themselves, heirs, assigns, personal representatives and next of kin, hereby release, indemnify
and hold harmless and agree not to hold liable, not to bring a suit or to take any type of legal action whatsoever against The Dance Studio in Oliver, its officers, agents, employees, members, managers, volunteers, sponsors, advertisers, and if applicable, owners and lessors of premises used to conduct The Dance
Studio in Oliver activities ("Releasees"), with respect to any and all injury, disability, death, or loss or damage to person or property, or loss of income or any expense suffered or incurred as a result of the student's participation in any activities The Dance Studio in Oliver whether arising from the negligence of the Releasees, including the intentional and unintentional actions of the Releasees, while in or upon the premises or any premises under the control and
supervision of The Dance Studio in Oliver, its owners and operators, or en route to or from any of the said premises. The undersigned shall indemnify,
defend, pay on behalf of and hold harmless the Releasees deem and against all loss, claims, demands, costs (including solicitor and client costs), damages, actions, suits, causes of actions or proceedings arising out of or in connection with the student’s participation and/or presence with The Dance Studio in
Oliver as well as the participation and presence of the undersigned, their agents, representatives, or next of kin. The undersigned and the student(s) agree
that it is their responsibility to manage the risks of participation with The Dance Studio in Oliver and to obtain such insurance coverage that they deem
suitable to address the risks of their participation adequately.

Studio Policies

Please read and initial each point listed below – Due to Covid 19, some of our policies have changed.

___ 1. All families must pay monthly installments for dance lessons. The season total will be divided into equal monthly payments for September to December and January to June.

___ 2. Initial registration is for the fall season, September to December, concluding with the Christmas show. Dancers that commit to a full year of classes will receive a discount of 10% off registration fees. Classes that are subsequently dropped will incur a cancellation fee of $100.00, and no refund of that month’s fees will be offered.

___ 3. A late charge of $25.00 will apply for monthly payments not received by the last day of the applicable month. A late charge of $50.00 will apply for accounts two months in arrears. If three payments are missed, the dancer will not be allowed to participate until payment is made.

___ 4. All payments are Non-transferable and Non-refundable.

___ 5. Tickets to the Christmas Show, the Year End Show, and a Video copy of the performance will be an additional cost to be determined later in the dance year. The cost of tickets to any performance is not included in the Tuition Fees.

___ 6. Dance attire and shoes are the responsibility of the dancer.

___ 7. Dancers with more than 5 unexcused absences risk removal from show choreography and may not be able to perform on stage in the recitals.

___ 8. I understand that if my child does not have proper clothing for class, they will be asked to sit and watch.

___ 9. Parents are responsible for arranging transportation for their children from school to the studio.

___ 10. Dancers’ temperatures may be taken at the door, and any dancer with a fever for any reason will not be permitted to stay for the class(es).

___ 11. Dancers will not be permitted to “lounge around” the change room. They must wait on chairs provided outside of their next classroom unless they need to go use the bathroom.

___ 12. There will be Zero Tolerance for any form of negative physical contact or for verbal or sexual harassment of any kind.

___ 13. Dancers may only keep their dance shoes, hair supplies, tights and bodysuits in the changeroom cubbies. Food may NOT be kept in the cubbies.

___ 14. Due to the months having varying numbers of classes, payments are calculated as the season’s total cost divided into equal parts. With registration, you are purchasing a space in the class(es). The monthly fee is not based on how many classes are booked for any specific month but on an even division of total fees.

___ 15. No filming of studio/teachers’ choreography will be posted on any social media without written consent, as it is the copyright of The Dance Studio in Oliver.

___ 16. The Dance Studio in Oliver accepts no liability for injury or illness under any circumstances.

___ 17. No Vaccine Passports will be required of any of the participants of TDS

___ 18. Dancers with any cold/flu symptoms will be asked to leave and not stay for class. Please keep your dancer home if they have any sort of illness.

___ 19. Parents of Dancers aged 6 and over are encouraged to wait for their Dancers in the upper hallway or at the top of the stairs so that Teachers and Dancers have visual confirmation that parents are present before the children are allowed to leave the actual building.
